Hillcrest Home provides a stimulating, structured daytime program of one-on-one and group activities to help individuals with dementia or physical challenges to maintain as much independence as possible for as long as possible. We’re partnered with Professionals for Aegis Therapy to provide an outstanding Rehab experience.

Most people hope they won’t need post-hospital care, but sometimes things happen outside of our control and short-term placement is necessary. After surgery, illness or injury, you want to get back to your life as quickly as possible. Our post-hospital services are your best way home after a hospital stay. We offer complex medical and rehabilitation services delivered by a team of professionals. We focus on caring for patients with many conditions including, but not limited to:

  • Pulmonary impairment

What to expect

Post-hospital or post-acute care is designed to improve your transition back to the community or a lower level of care. As a post-acute patient, you no longer need the acute care setting of the hospital which includes around-the-clock physicians, surgery, emergency and ICU services, but still need a high level of clinical and rehabilitation care to hasten your recuperation. This may include skilled nursing, rehabilitation, dietary management, social services and discharge planning. All care is directed by your physician, and our team will be in regular contact with you, your family and your physician(s).

With a focus on short-term rehabilitation, most patients are discharged home within four to six weeks. For those needing a little more time or who choose to stay a little longer to get a little stronger, we have intermediate- and long-term care services at times, depending on the demand for these services.

Payment options

We are Medicare-certified for post-hospital or post-acute care.  In addition, we accept some, but not all, Medicare replacement plans, most managed care and commercial insurance, as well as private payment. We also accept Medicaid.

Your post-hospital medical and rehabilitation care is an important part of your recovery. It requires the dedicated teamwork of all those involved in a patient’s care to treat the whole person and provide the greatest opportunity for you to achieve your discharge goals. The continued effort of our post-acute center’s nursing and therapy team provides the necessary ingredients and support for a successful discharge back home. Our team includes:

Physical Therapy We have a full staff of Licensed Physical Therapists and Physical Therapy Assistants. Many residents receive therapy twice daily depending on the resident’s needs and tolerance. Services provided include but are not limited to:

  • Rehabilitative exercises
  • Ambulation training
  • Moist heat packs
  • Ultrasound
  • Family instruction

When a resident is returning home, we work to assist in overcoming obstacles in the home environment. We teach each resident and family member stair negotiation, transfer techniques, ambulation skills, and address any special circumstances involved in returning home.

Occupational Therapy evaluates and treats problems with activities of daily living including but not limited to:

  • Eating
  • Dressing
  • Hygiene
  • Bathing
  • Homemaking
  • Functional mobility improvement includes getting in and out of a chair or car and on and off a toilet

The purpose of the training provided is to help the resident become as independent as possible.

Home Evaluation – As part of the discharge planning process, The Occupational Therapist conducts a home evaluation including equipment recommendations which will help the resident maintain maximum independence in the home environment.

Speech Therapy

A Licensed Speech Pathologist provides speech, language and swallowing services. The Speech Pathologist evaluates the resident referred by the physician and develops a rehabilitation plan. The plan defines the type and frequency of therapy required to maximize the resident’s recovery.

Psychological services

Adjusting to the many life changes during a nursing home admission is never easy. Counseling services are available if ordered by the physician to assist with psycho-social adjustments. Some of the reasons a physician may order Psychological services include:

  • Programs to eliminate the need for chemical or physical restraints
  • Behavior problems
  • Grief counseling
  • Depression
  • Conflict resolution
  • Anxiety
